Cannabinoid formulations chemically modified to dissolve in water rather than oil, enabling intravenous (IV) administration. Cannabis is naturally oil-based, making it impossible to inject directly into veins. Converting cannabinoids to esters makes them water-soluble. A US patent exists for this conversion process. Dr. David Allen predicts that water-soluble cannabinoids will revolutionise emergency medicine โ with ambulances carrying IV cannabinoid solutions for heart attacks, strokes, spinal injuries, and other acute conditions.

Water-soluble cannabinoids were introduced in Episode 16 by Dr. David Allen, a retired heart surgeon. He explained the fundamental challenge: "Cannabis has always been hard to study because it's an oil. It's not water soluble. You can't inject oil into somebody's vein โ it's going to kill them." The solution is converting cannabinoids to esters, making them compatible with IV delivery. Allen's prediction was striking in its scope: "When the ambulance picks you up, no matter what kind of disease you have โ heart attack, stroke, gunshot wound โ they will start an IV and give you water-soluble cannabinoids to decrease oxidative metabolism and help you survive." He specifically cited reperfusion injury in heart surgery as a key application, where cannabinoids could prevent tissue damage when blood flow is restored.
